Position Review & Reclassification Request

The purpose of the Position Review & Reclassification form is to submit a position review and reclassification request for a current employee in your unit. Submission of the request will create an RT ticket sent to the EOHR team for review and processing. Please include any documents you may have related to the request. Additional information or approval may be requested/required based on the request. Please fill out the form to the best of your knowledge.

Required Documentation

Prior to filling out this form, please have the below documentation prepared that is applicable to your request:

  • If this request is for a position review (professional staff), EOHR requires both the PSPD template and the Position review form in order to process this request.
  • If this request is for reclassification (classified staff), EOHR requires that the Classified Staff Position Review Questionnaire (docx) be filled out (but not yet signed) in order to process this request. EOHR will handle the formal submission of the form request to HR Compensation Office.
