Required Documentation
Prior to filling out this form, please have the below documentation prepared that is applicable to your request:
- If this request is for a position review (professional staff), EOHR requires both the PSPD template and the Position review form in order to process this request.
- If this request is for reclassification (classified staff), EOHR requires that the Classified Staff Position Review Questionnaire (docx) be filled out (but not yet signed) in order to process this request. EOHR will handle the formal submission of the form request to HR Compensation Office.

- If this request was employee initiated, the EOHR requires correspondence documentation of schedule change request (the employee initiation and manager approval).
- If this request was supervisor/manager initiated, the EOHR requires formal notification of schedule change (from supervisor/manager to employee).

Best Practices and Instructions
Before You Fill Out the Form
Ensure you have already discussed the details and purpose of the position with your Unit Approver.
Only Authorized Requestors may submit this form on behalf of their respective unit. For urgent requests when the Authorized Requestor is unavailable to submit, email eohr@uw.edu for guidance and support.
Ensure you have or have completed the following:
- Position Description
- Purpose/reason for the new position: Full background and details can be provided as a document and uploaded in the form, or the text copied and pasted in the Additional Information field.
- Director Review: Documentation that this action has been discussed and approved by the unit Director. (When submitted by someone other than the director.)
- Student Details: Student Name, UWNetID, ID Number, Citizenship Status.
- Academic Student Employees (ASEs): These student hires require a consultation with a member of the EO HR Team.
- Work Study Students: These student hires require a consultation with a member of the EO HR Team.
While You Fill Out the Form
Complete and detailed information will ensure a more efficient review/approval process by the EOHR Team. Incomplete forms cannot be saved in order to return to it.
- Required Fields: Fields are denoted and must be completed.
- Pay Information: Propose an hourly rate for hourly position only if the rate is intended to by higher than minimum wage.
- Work-Study Student: Be sure to select Yes. EOHR will provide follow-up information about the Work-Study Hiring process once the request is approved.
- Documentation Upload: Upload the position description, correspondence, and any associated forms or documents.
- Additional Information: Provide any information that provides additional context not captured in other fields on the form.
